Pain management following a total Knee replacement

A 67-year-old client had an epidural catheter inserted for pain management following a total knee replacement. After successful treatment of an episode of hypotension in the postanesthesia care unit (PACU), he was transferred to the medical-surgical unit.

The registered nurses assessed the client and delegated direct care to an LPN, although the LPN denied this.

Three hours later, the client experienced nausea and vomiting. The client was found cyanotic and unresponsive 10 minutes later and subsequently died of anoxic encephalopathy.

Many facts were disputed among the numerous codefendants and there was poor documentation (Nurses Service Organization, 2012).

Case Study Questions

  1. What priority assessments were missed, and how could this have been avoided?
  2. What steps of the nursing process were done inappropriately or missed completely, and why?
  3. What steps of the delegation were inappropriate, and why?
  4. What could have been done to prevent this outcome?

What are the seven (7) patient rights?

What are the seven (7) patient rights? With each right give an example of how an EN could ensure that the patient has access or is afforded these rights.  Please give me some practical examples for each of these rights 🙂

Access

I need to ensure the patient has a right to health care and access to healthcare services/treatments that meets their needs. Access to health care is a basic right for every person in Australia. 

Safety

All patients have the right to safe and high-quality health care that meet national standards. Patients are to be cared for in an environment that makes them feel safe. If the patient doesn’t feel like they are being heard or we may have missed in their care they should feel they can voice their concerns. 

Communication

We need to ensure that patients understand the information they are provided. Patients need to be able to understand the conversation clearly and have access to an interpreter if needed. Communication is important with patients, and they should feel they can share information. 

Participation

Patients have a right to be apart off all decisions and choices about care. Participation by patients in their health care is encouraged by engaging the patient in discussions about treatment options. 

Privacy

A right to privacy and confidentiality of provided information. Privacy requires that all participants be sensitive to the privacy needs of patients and ensuring that the patient’s information is only shared with other appropriate health professionals when necessary. 

Comment

A right to comment on care and having concerns addressed. The patients need to feel like they have been listened too. They should feel like they can make a comment. By being attentive to the concerns of the patient this is encouraging them to participate in a two-way communication. 

Respect

All clinical staff are mindful of a patient’s environment and background and providing health care and advice that is appropriate to the patient.

Richard is a 50-year-old male with nasal congestion, sneezing, rhinorrhea, and postnasal drainage.

Richard is a 50-year-old male with nasal congestion, sneezing, rhinorrhea, and postnasal drainage. Richard has struggled with an itchy nose, eyes, palate, and ears for 5 days. As you check his ears and throat for redness and inflammation, you notice him touch his fingers to the bridge of his nose to press and rub there.

He says he’s taken Mucinex OTC the past 2 nights to help him breathe while he sleeps.

When you ask if the Mucinex has helped at all, he sneers slightly and gestures that the improvement is only minimal. Richard is alert and oriented. He has pale, boggy nasal mucosa with clear thin secretions and enlarged nasal turbinates, which obstruct airway flow but his lungs are clear.

His tonsils are not enlarged but his throat is mildly erythematous

NB

**You will need to put in (MAKE UP) the missing information in the note (some of the information you will have to MAKE UP ie meds, hx, parts of the ROS and PE). I’m looking to make sure you know what information to include.

In the Assessment/Plan, you will document your differential diagnoses as per the assignment.
