Physical examination

A 20-year-old woman notices a swelling in her neck, just to the left of the midline. Physical examination suggests a nontender 1.5-cm thyroid lesion, and thyroid function tests reveal normal serum thyroid hormone levels.

A radioisotope scintiscan demonstrates very little uptake of the iodine in the nodule, consistent with a nonfunctioning nodule, or “cold” nodule.

The Decision-making framework

Huang is a registered nurse and has recently commenced employment in a regional South Australian town in a large nursing home.

During his shifts he is the only registered nurse in charge of 68 residents, 35 of whom have high care needs.
The majority of high care residents have been diagnosed with dementia or related disorders.

Most of his shift involves completing medication rounds through various wards and documenting records.

The nursing home is constantly plagued with understaffing issues. Huang has little time to attend to the individual needs of residents. Assistants in Nursing provide most of the care to the residents in both the low care and high care wards.
During his first week at the nursing home Huang has witnessed the following incidents in the 12-bed high care dementia ward:
• At any given time up to eight ‘mobile’ residents in the ward are restrained in chairs (the other four are restricted to bed) for lengthy periods of time.

Two of the more alert residents display signs of psychological distress as a result of the restraint. One of the patients continually asks to have the restraint removed.

Huang has been told the restraints are necessary to prevent falls as there is not enough staff to watch the residents and ensure their safety.

• One aggressive and noisy resident is restrained at all times and is often moved to a secured room for long periods of time, particularly during peak visiting hours.

Huang has been told that visitors have complained about the resident’s constant screaming and that his noise also upsets other residents in the ward and therefore the seclusion of this patient is necessary for the comfort of others.

Medical-Surgical Nursing

Harding: Lewis’s Medical-Surgical Nursing, 11th Edition. Dysrhythmia and Pacemaker (Ch. 35) A 38-year-old female is admitted to the emergency department after “feeling palpitations and almost fainting.” She states that this started a few hours ago and got progressively worse as she tried to get ready for work.

The ECG reveals nonconducted P waves without progressive PR intervals.

On conducted beats, the PR interval is constant, with a ratio of 3:1 (three P waves to one conducted QRS interval). The ventricular rate ranges from 50 to 60.

The PR interval is slightly prolonged; the QRS complex is greater than 0.12 seconds. Blood pressure is 108/54 mm hg, temperature 97.8°F (36.6°C), respirations 18 breaths/minute.

Currently she is not feeling faint but reports feeling occasional palpitations. She reports that she has felt extremely tired for the past few days. The patient states that she has a family history of heart disease and that her mother had a heart attack at age 35. She denies having any chest pain or tightness.

The patient states that she has smoked 1 pack of cigarettes a day for years but stopped smoking a month ago. Her skin is warm and dry, and capillary refill is brisk.

The patient is assisted back to bed. The nurse notes that the ECG monitor shows that the patient’s heart rate is at 40 beats/minute and she is now in third degree heart block.
