Review the person’s care plan and records and identify: a. the primary health care service model used by the person b. the roles and professional expertise of the members of the interdisciplinary health care team involved in the person’s care. 2. Using the primary health care service used by the individual, provide a brief summary of the health issues which are applicable to the community or target group of the primary health care service.
