She needs assistance with feeding, bathing, toileting, and dressing. When she is oriented, she is pleasant and cooperative. When she is confused, she yells and upsets the other patients. The patient was placed in a private room across from the nurses’ station, but she complains of the noise and constant commotion. The staff agrees that the patient needs to be close to the nurses’ station for better monitoring. How can the nursing staff make the environment safer and less stressful for the patient?
