Increase in medical errors

Noticeable lethargy in newborn

A newborn infant was discharged from the postpartum unit despite concerns from the parents of difficulty breathing, feeding and noticeable lethargy. The parents return with the infant to the hospital’s emergency department, three days later and a diagnosis confirmed hypoxic ischemic encephalopathy due to severe hypoglycemia and cortical blindness. A root cause analysis revealed the infant’s blood sugars were not tested despite the mother having type 1 diabetes and a feeding assessment Was not completed. The Quality Improvement Specialist decided to implement a new checklist upon discharge as an improvement initiative. Describe how you would implement this change using Plan-Do-Study-Act (PDSA). Reminder: just repeating the Plan-Do-Study-Act (PDSA) steps will not earn full marks, you must apply them to practice and be soecifis

(Use headings for Plan-Do-Study-Act (PDSA) and provide a short paragraph for each part)

Which mode of inheritance does the pedigree illustrate?

This pedigree illustrates a single-gene heritable disease named oreotitis. I. .I\ IOIIIOOI a) Which mode of inheritance does the pedigree illustrate? Briefly (2-3 sentences) explain your answer. HINT: think about what type of inheritance it ISN’T as well as what type it IS. b) Define some alleles for oreotitis. C) Using those defined alleles, provide a oreotitis genotype for the individual highlighted by the arrow. 2. You have a tetrahybrid (so that’s an individual who is heterozygous for -YOU GUESSED IT – four genes) lily that has pink blooms, long stamen, round pollen, and thick roots. Each of these genes exists on different chromosomes within the genome. a) If you self-fertilize this lily plant, what are the odds that an offspring would be homozygous recessive for all four traits? b) If you self-fertilize this lily plant, what is the probability that an offspring would have long stamen with oval pollen?

Managing a critical incident

What is the policy and procedure for managing a critical incident where a resident falls and injures his head ? Explain why its important to understand what happened before , during and after the incident.

Magnetic resonance imaging of head

“A 59-year-old Hispanic man presented with right upper and lower extremity weakness, associated with facial drop and slurred speech starting 2 hours before the presentation. He denied visual disturbance, headache, chest pain, palpitations, dyspnea, dysphagia, fever, dizziness, loss of consciousness, bowel or urinary incontinence, or trauma. His medical history was significant for uncontrolled type 2 diabetes mellitus, hypertension, hyperlipidemia, and benign prostatic hypertrophy. Social history included cigarette smoking (1 pack per day for 20 years) and alcohol intake of 3 to 4 beers daily. Family history was not significant, and he did not remember his medications. In the emergency department, his vital signs were stable. His physical examination was remarkable for right-sided facial droop, dysarthria, and right-sided hemiplegia. The rest of the examination findings were insignificant. His Glasgow Coma Scale was calculated as 9. Initial Computed tomography angiogram of head and neck reported no acute intracranial findings. The neurology team was consulted, and intravenous recombinant tissue plasminogen activator (t-PA) was administered along with high-intensity statin therapy. The patient was admitted to the intensive care unit where his hemodynamics were monitored for 24 hours and later transferred to the telemetry unit. MRI of the head revealed an acute 1.7-cm infarct of the left periventricular white matter and posterior left basal ganglia.”

Question:

  1. Why do you want to know if the patient has “visual disturbance, headache, chest pain, palpitations, dyspnea, dysphagia, fever, dizziness, loss of consciousness, bowel or urinary incontinence, or trauma”.
  2. Based on the following statement “magnetic resonance imaging of the head revealed an acute 1.7-cm infarct of the left periventricular white matter and posterior left basal ganglia.” Which side of the patient’s body would be affected by this statement?

Compiling list of medications

A 73-year old female scheduled as a new patient arrives with all of her prescription and OTC (over-the-counter) medication as well as vitamins in a small bag.
