Noticeable lethargy in newborn
A newborn infant was discharged from the postpartum unit despite concerns from the parents of difficulty breathing, feeding and noticeable lethargy. The parents return with the infant to the hospital’s emergency department, three days later and a diagnosis confirmed hypoxic ischemic encephalopathy due to severe hypoglycemia and cortical blindness. A root cause analysis revealed the infant’s blood sugars were not tested despite the mother having type 1 diabetes and a feeding assessment Was not completed.
