Medical geriatric diabetic unit

Kate, RN, BSN, has recently been promoted to clinical team coordinator in the medical geriatric diabetic unit. The geriatric, diabetic population has been one of those patient groups selected for testing out new models of care. The proposed model would emphasize earlier engagement in patient care management and the longer-term continuum of care service structure for delivering care to this population of patients. Kate has been asked. to put together a small team of nursing colleagues to directly effect a reduction in hospital inpatient admissions, a higher level of quality of service, and a stronger positive impact on the lives of geriatric diabetic patients.
