What additional information do you need to complete the capstone project paper and what is your plan to attain that?

My capstone project is preventing hospital readmission for stroke patients after being discharged from the hospital and will be utilizing home health care with RN and therapy services. In order to prevent rehospitalization, my focus is on patient education, medication management, and making lifestyle changes
