A nurse is assisting a client with showering. The client is elderly and unstable on his feet he doesn’t like to sit while bathing and often needs to be reminded by the nursing staff not to stand up from the bath seat. The nurse is distracted by a text message on her phone and doesn’t notice the client attempting to stand. Before she can act, the client has slipped forward and hurt himself.
