Describe how to generate positive change related to priority problem. Include the change theory utilized and strategies/interventions formulated to accomplish your goal.
- Discuss how you would proceed (actions/interventions/strategies) regarding the plan for change for your highest priority problem. How would you improve quality? How would you facilitate a change? What change theory (p. 62-64) would you use?
- How does the change theory you selected apply specifically to your plan for change with your highest priority problem?
- Describe a creative teaching strategy(s) you would utilize to accomplish your goal. How would you present this to the staff? Hospital management?
- Would you use handouts, posters, flyers, video, group activity? Provide a visual of your strategy. Get creative.
- State the goal of this quality improvement project (measurable).
- Include a plan for evaluating the change occurring. Be sure to start with the overall QI goal and then include how you will/plan to measure the change.
- How would you measure the change? (i.e., Change over three time points? One month after implementation?, etc.)
- The goal of your plan MUST be measurable!
- Example: Goal = Decrease hospital-acquired pressure ulcers by 30% in two months.
- List specific QI measures which will evaluate the impact of your planned change/intervention. QI measures are the data you plan to collect.
- How does this data verify you have met your goal? You may have several QI measures depending on the problem you chose.
- Example: QI Measures/Indicators:
- Number of pressure ulcers two months prior to implementation.
- Number of pressure ulcers two months after implementation.
- Number of patients who received Meplex dressing.
