Using the case study of a patient with mental health such as depression you worked with in the past as a mental health nurse. 1. construct a formulation of service user using 5ps approach explaining the cause of the mental illness

2.use this example to demonstrate the ability to apply and explain the use of biological, psychological and social interventions with this client.

3.Include risk management plan and  ensure that you make reference to service user and carer involvement

4. Highlight how you worked using care programme approach (CPA) tool in evaluating intervention or any other tool in evaluating nursing intervention
