Normal saline bolus and insulin drip

The physician orders normal saline bolus at 1000mL/30 minutes, insulin drip at 5 units per hour. The insulin is supplied in 50 units in 100mL. What rate should the pump be set for the normal saline bolus and the insulin drip? How often should the patient have blood sugar checked?
