Review of systems

When you have a 32-years old female patient complaining of tiredness, hair falling out, gain 30 pounds in the past years, but had a decrease appetite and the female does not sleep well and she is depressed. What are all the Review of Systems(ROS) related to the cubic centimeter(CC) and would the nursing practitioner assess, what are at least five different things could be wrong with this patient. What system would the Thyroid be assess in? What labs or diagnostic test ? try to make sure I am on the right track.
