Rubric to grade an oral presentation

how do I develop a rubric to grade an oral presentation by student and be sure to have at least 3 criteria A,B,C D,and F as possibli have to make sure i include what over score/ gradeis is as part of the document .this should be created in a table or form .what i.creat and is included in the paper as appendix A ( not as a separate document)
