Skin structure infection

A 38-year-old female with complicated skin and skin structure infection (cSSSI). Her height is 5’4” and her weight is 154 lbs. Rx: Administer 4mg/kg daptomycin in a 50mL intravenous infusion bag containing 0.9% sodium chloride for a period of 30 minutes. Final concentration to be 50mg/mL; there are two formulations that have differences concerning storage/reconstitution – Cubicin vs. Cubicin RF.
