Surgical myocardial revascularization

IMPRESSION: This is a 49-year-old male who has significant multi-vessel coronary artery disease. He has atypical anginal symptoms, suspected to be secondary to his type 1 diabetes mellitus. Still, it is believed that he is at risk for ischemia. To reduce this risk, surgical myocardial revascularization is recommended.

 

The patient is given complete details about the procedure, the risks, and the benefits. We also discuss alternative treatments that may be viable. The patient signs the informed consent and the surgery is scheduled.

 

PLAN: Proceed with coronary artery bypass graft operation utilizing the left internal mammary artery as a conduit to the left anterior descending. The remaining conduit will come from the greater saphenous veins.
