Carcinoma of the transverse colon

This 45-year-old male underwent colon resection for carcinoma of the transverse colon—the physician’s progress notes on postoperative day two state anemia. Hemoglobin and hematocrit levels dropped significantly after surgery, and a blood transfusion was ordered. Explain how is anemia coded?

Fracture of the left hip at The femoral neck

Jeanine Porter is a 75-year-old woman who was admitted to the hospital after she fell at home and sustained a fracture of the left hip at the femoral neck. She is widowed and lives with her daughter and son-in-law and their three children, ages 15, 12, and 4.

She has a 50 pack-a-year smoking history and denies alcohol use. She has severe osteoarthritis of her knees and coronary artery disease (CAD).

She underwent coronary artery bypass grafting (CABG) 9 months ago. Since her surgery, she has engaged in minimal exercises at home.

Mrs. Porter underwent a total hip replacement. Her postoperative period was unremarkable, and she was discharged home after 10 days. At a home health visit 2 weeks after discharge, Mrs. Porter was in the living room, seated in a rocking chair. Her walker was across the room.

The room smelled of smoke, as well as open bags of chips and several open soft drink cans. The floor was littered with toy trains and cars, and the sofa was full of unfolded, clean laundry. In the kitchen, the countertops and sink were loaded with dirty dishes, pots, and pans.

Mrs. Porter said, “My daughter is really doing her best. I guess I am just too much for her…what with the children and all.” Mrs. Porter’s daughter replied, “I can handle it, I just don’t get any help from anyone, including my mother.”

Client with heartburn

A 45-year-old male client with heartburn comes to the nurse practitioner (N P) stating he wakes often at night with coughing and burning he describes as heartburn. He drinks 1-2 alcoholic beverages per evening, usually with distilled liquor, and then wine with dinner.

He eats heavy dinners with dessert.

He prefers mint ice cream and coffee for dessert. The NP prescribes a regimen with omeprazole (Prilosec) every morning and Maaloz 30 mL qid. How will you instruct the client about gastroesophageal reflux disease and its relationship to symptoms?

What will you instruct the client about his diet, particularly the evening meal? How will you instruct the client about his medications?

Taking notes as a way to anchor your learning

Directions: It is generally agreed that taking notes as a way to anchor your learning. That is what this journal assignment is all about helping you to anchor the readings in your mind.  In this course, you are asked to write a reflection of your “take away” from the assigned readings.

Please pay special attention to the directions for each journal assignment. You will be asked to address specific learning objectives for the assigned week and reflect upon your personal learning as well.

Note that these journals are NOT public. The faculty in the course have access to them but your student colleagues do not.

Please record your “aha” moments or other insights that you may have gained about yourself or your organization.  The minimal length for the journal entry is TWO substantive paragraph, but you are encouraged to write as much as you wish. No citations are required in your journal entries but including outside readings is encouraged.

Week 11 Journal: Write about your reaction to the course content, the readings, and your own leadership style. Reflect on your personal growth, understanding, and development as a leader while briefly addressing this week’s Learning Objective 2.

 

Online crisis intervention

Take a position, for or against. Is it better/more important to have online crisis intervention, or face-to-face crisis intervention, and why? Think of yourself in a crisis location where both are available.

Rules of social interaction

Rules of social interaction (Ch 4); as we know, in public spaces we see the struggle to connect and to learn from other through socialization and everyday social interaction. So, one example, might be how we see women and men in public spaces. In the textbook, it shares the example of women and men on a walk way, women are more likely to be “catcalled”. But, this goes beyond gender, in how we see the interactions in regards to race. Goffman asks, “What types of behavioral cures and signs make up the vocabulary of public interaction?” So, sharing your thoughts, do you think that the advent of the internet and our technological changes have better helped us to connect and interact with each other or that we are seeing more and more struggles to interact with each other? Are these changes causing more social trouble, and social problems?

Epididymitis and Testicular torsion

Epididymitis and testicular torsion are both causes of scrotal pain. Testicular torsion requires prompt recognition and surgical intervention to avoid testicular damage. Epididymitis is easily managed with antibiotics. Differentiation between the two disorders is important. Review the case and answer the questions

 

Case: A 15-year-old presents to the emergency department complaining of sudden onset of right-sided scrotal and groin pain. He states the pain is constant and extreme.

He said it started about 2 hours ago during basketball practice at school, but he denies any trauma to the scrotum. He is worried that he has a sexually transmitted infection as he just started having sexual intercourse with his girlfriend and he did not use condoms. He says he feels nauseated and thinks he is going to vomit.

He said he noticed this same pain once before, but it went away within a few seconds and he did not tell anyone. Vital signs: Temperature 98.9° F; pulse 100 beats per minute; respirations 20 breaths per minute; blood pressure 120/70 mmHg. Physical examination reveals a healthy-appearing adolescent who is anxious and grimacing.

The patient has edema and erythema of his right scrotum. His scrotum is tender, and the patient only tolerated superficial palpation.
