  • What are the effects of DIC on the Partial thromboplastin time (PTT)?
  • What are the effects of DIC on Platelets, fibrin split products (FSPs)?
  • What are the effects of DIC on Fibrin degradation products (FDPs)?
  • interprofessional and nursing management of disseminated intravascular coagulation.

Include:

  • Describe goals and outcomes related to this topic.
  • Use the SMART acronym to describe expected outcomes:
  • Specific to the patient and the problem
  • Measurable (generally, quantifiable with a number, for example, if you are providing patient education, the patient will identify and teach-back 3 things you taught, or if activity intolerance is the problem, state that the patient will be able to ambulate an extra 25 feet each day)
  • Achievable
  • Realistic
  • Timeframe able to be achieved in a reasonable amount of time.

Health teaching plan for this client

SCENARIO; What comprehensive health teaching plan for this client / Scenario.ASSESSMENTLearning needs

Daniel Wong is an 8 month old infant who was seen in the ER this morning after having a febrile seizure. Daniel was given acetaminophen and is now afebrile. He has been assessed by the physician and cleared to be discharged after teaching has been done with his parents regarding febrile seizures. Daniel’s parents are both new to the country and have very limited understanding of English. They appear very anxious and concerned regarding what has happened but communication is very difficult. Daniel’s mother keeps motioning to a container in her purse that appears to be an herbal remedy and pointing at Daniel (motioning to give him some). You are required to provide discharge teaching regarding febrile seizures to the Wongs.

  • Ensure your teaching plan is specific to client’s age, developmental level, educational level, basic understanding etc.
  • Include with your teaching plan additional information  may give to client or any links or resources  to ensure they are appropriate for the client’s level of education and understanding
  • Evaluate the teaching plan and Include next steps based on the evaluation

  • Preferred methods of instruction

  • Any barriers to
    learning
  • Family support

  • Motivation to learning

OBJECTIVE

Note;

SMART Goals
Ensure
Specific
Measurable
Achievable
Realistic
Time Sensitive

  • One SMART goal
    for cognitive

  • One SMART goal
    for affective

  • One SMART goal
    for psychomotor

PLANNING

•objectives based on priority
• 3 the domains of learning
• specific and realistic timelines for completion
•What are the rationale

In this column based on the objectives what steps is going to take to achieve for each SMART goal.; provide a rationale  of why it is important.

In this section is need  one goal from each domain (cognitive, affective, and psychomotor) with rationale.  Rationale requires

IMPLEMENTATION

Describe details regarding how teaching to be delivered for each domain 3 in total
Environment
Participants
Time of day
Materials needed
Note if teaching to be delivered in one session or over several sessions including length of time for sessions

Ensure for each plan (3 in total)  how to perform the teaching and why (rationale.)

EVALUATION

  • How will it be determined if teaching plan was successful?
  • Refer to each specific objective 3 in total.
  • Note next steps and future needs based on the evaluation of the teaching. State if next steps should or should not take place.

Explain how each goal was met and if there is a need for additional steps. If additional steps are required explain why if NOT explain why.

An outpost nurse

Althea works as an outpost nurse on a remote aboriginal reserve in northern Ontario. For the past two months, she has been working hard to serve the numerous needs of the community. Being only one of two healthcare providers in the isolated community of 1300 people, Althea often feels overwhelmed by the responsibility of caring for so many people. To make matters worse, the people living in the reserve seem to be sicker than any other patients Althea has ever cared for. Many of the children are malnourished, as their parents cannot afford to buy the extremely expensive ‘healthy foods’ offered at local stores, and severe illnesses like tuberculosis, which have been eradicated elsewhere in Canada, runs rampant on the reserve. Visiting the homes of her patients, Althea also sees that the living conditions for people on the reserve are less than adequate, as many homes have mold, are damaged, and lack plumbing. Both through her nursing practice and speaking to the people who live on the reserve, Althea feels she has started to begin to understand what the barriers and challenges of living healthy a healthy life on a northern remote reserve are. She wants to do something to help her patients but is not sure where to start.

1. Do you feel Althea truly understands the experience and meaning of health among her patients? Why or why not? (500- 1000 WORDS)

Anatomic pathology laboratory information system

“Currently Kaiser Permanente (KP) laboratories utilize Cerner

CoPathPlus (CPP) as their anatomic pathology laboratory information system. CPP does not have native integration with KP Health Connect (KPHC), and interfaces were built out in each market to provide integration.” (Epic, 2020, p. 3). The Kaiser system laboratory uses Cerner and CoPath for their labs and a form of Epic called Health Connect (KPHC) by Kaiser. Cerner has announced they are retiring CPP. So all of Kaiser has decided to take Beaker AP which is owned and integrates with Epic and KPHC.
Speaking with the Histology and Pathology departments currently, they have identified many gaps that are occurring with CoPath that they would like changed/ fixed when Beaker gets put into place. Beaker has been used by many other labs outside of Kaiser for years and reports that it works well. Phase 1, which is happening now, includes the scope, concept, and collaborative build and deployment in 2 markets. Colorado, which is where I am located, is one of the 2 markets. Nursing informatics, Lab informatics, IT, project management, and executive leadership are involved in phase 1. Implementing Beaker will allow the anatomic pathology laboratory to be able to have orders and results interface with health connect that is currently in use. This software will be updated with the quarterly Epic updates and there are no plans to retire this software. The solution is to change to software that already integrates with their current system since CoPath is being retired.

Blood pressure control

Describe the physiology of blood pressure control. Causes of Primary

Hypertension may include overactivity of the SNS; overactivity of the RAAS; alterations in other neurohumoral mediators of blood volume and vasomotor tone such as ANP, BNP, and adrenomedullin; inflammation; a complex interaction involving insulin resistance and endothelial function; and obesity-related hormonal changes
Select 3 of the above potential causes and explain the pathophysiologic mechanisms of primary hypertension.
Response #1 Post a response to one peer in your discussion group. In the post identify the organ damage that can occur as a result of Hypertension. Describe the pathophysiologic process of the organ damage.
Response #2 Orthostatic hypotension can occur as an adverse effect of medications used to manage hypertension. Go back to your initial post and describe the pathophysiologic process that causes orthostatic hypotension.

The technological advance

Almost every medical or technological advance seems to be accompanied

by new and vexing ethical dilemmas. Yet, the United States has no structure in place to resolve such issues. Should the federal or state governments take responsibility for ethical decision-making and for protecting the public? If neither, what might be other options for providing ethical guidelines and oversight?

Post-operative infection

The patient had surgery last week and presents with a post-operative infection. Which modifier would be used?
