Comprehensive pediatrics clinic

Jane brings her four-year-old named Tim to a comprehensive pediatrics clinic in order to assess his health status. Jane reports that Tim is very curious and smart and gets along socially with other children. Jane says that Tim’s preschool teacher suggested she take him to the doctor for a health screening because she has noticed his stools looking oily when he uses the bathroom.

Patient History:
Tim’s mother Jane is a former drug user and has no family in the area. For these reasons, along with the socioeconomic status of his household (below the poverty line), Tim had a difficult first few years of life. However, recently his mom got help from social services and has been drug-free for six months. They are living in a new rental home and Tim is adjusting well to his new living situation but is still struggling at school. He recently had several accidents and so they temporarily put him in pull-up diapers. He has also shown to be hesitant in engaging in physical activity and seems more tired and exhausted than a typical four-year-old should be.

Signs of prolapse of the intervertebral disc

  1. Peter Parker, is a 60-year-old male who has been smoking for the past 35 years. He came to the hospital with breathlessness, cough, and chest pain radiating to his left shoulder. He was admitted to the pulmonary ICU as his Oxygen saturation was fluctuating. His Emergency Chest X-ray showed signs of COPD. Along with regular blood and cardiac investigations, he underwent a coronary angiogram which reported 2 blocks. The patient is set for further cardiac management once the Oxygen saturations increase.

    His MRI showed signs of prolapse of the intervertebral disc with compression of L4-L5 nerve roots. He is now being prepared for surgical laminectomy.

 

  1. 15-year-old Diana Prince met with an accident while riding her bicycle yesterday. The paramedic reported blunt trauma to the pelvis. Her Blood pressure was falling and hence she was shifted to the ICU and transfused 3 units of Packed RBS. Her BP is 110/70 mm of Hg now. The CT scan reported an open-book fracture of the pelvis. Blood reports from this morning indicated a rise in Serum Creatinine level from 1.1mg/dl to 5.2mg/dl. She was diagnosed with Hypovolemia induced Acute Kidney Injury with a fractured pelvis. She is now being monitored by a team of Nephrologists, Orthopaedic, and Trauma surgeons.
  2. 32-year-old Wade Wilson, a known case of HIV, came to the hospital with pedal edema for 2weeks. He says he was being treated for HIV when he recently started noticing foamy urine. After a renal biopsy, he was diagnosed with Nephrotic syndrome secondary to HIV. Heis now on medical treatment for the same.
  3. 57-year-old Bruce Banner came to the hospital with complaints of blurred vision for 2 months. He has been on oral hypoglycemic drugs for 10 years now as he despises insulin injections. He was diagnosed with diabetic retinopathy and is being managed by an Ophthalmologist and an Endocrinologist.
  4. 45-year-old Steve Rogers came to the hospital with generalized pain all over the body, loss of weight for 3 months, and loss of appetite. He underwent a PET CT scan and was diagnosed with a Parathyroid tumor. His blood tests showed increased levels of parathyroid hormone and Calcium. The Orthopedic surgeon has requested him to get a Bone Densitometry test done as he suspects osteoporosis secondary to hyperparathyroidism is a reason for the generalized pain. He is currently posted for surgical excision of the tumor.

 

  1. Logan, a 7-year-old boy who was diagnosed with cystic fibrosis at birth was accompanied to the hospital by his stepmother with complaints of high fever with chills/rigors, cough, and weakness. The stepmother claims that Logan looks malnourished because she forgets packing him breakfast. Investigations confirmed Lower respiratory tract infection and malnutrition. He is now being managed by the pediatrics department, pulmonologists, and gastroenterologists in the respiratory ICU.
  2. Sheila, a 35-year-old female who was on and off chemotherapy for 2 years for Carcinoma Cervix, came to the hospital with paralysis of the left side of the body. An MRI scan of the brain showed metastasis of the tumor to the brain and spinal cord. She is now being managed by surgical oncologists.
