Signs of prolapse of the intervertebral disc

  1. Peter Parker, is a 60-year-old male who has been smoking for the past 35 years. He came to the hospital with breathlessness, cough, and chest pain radiating to his left shoulder. He was admitted to the pulmonary ICU as his Oxygen saturation was fluctuating. His Emergency Chest X-ray showed signs of COPD. Along with regular blood and cardiac investigations, he underwent a coronary angiogram which reported 2 blocks. The patient is set for further cardiac management once the Oxygen saturations increase.
  2. 30-year-old Bruce Wayne came to the hospital with a low back ache with tingling and numbness radiating from the back of his thigh to his little toe. He drives a truck and distributes large containers of beef in Melbourne.
His MRI showed signs of prolapse of the intervertebral disc with compression of L4-L5 nerve roots. He is now being prepared for surgical laminectomy.

  1. 15-year-old Diana Prince met with an accident while riding her bicycle yesterday. The paramedic reported blunt trauma to the pelvis. Her Blood pressure was falling and hence she was shifted to the ICU and transfused 3 units of Packed RBS. Her BP is 110/70 mm of Hg now. The CT scan reported an open-book fracture of the pelvis. Blood reports from this morning indicated a rise in Serum Creatinine level from 1.1mg/dl to 5.2mg/dl. She was diagnosed with Hypovolemia induced Acute Kidney Injury with a fractured pelvis. She is now being monitored by a team of Nephrologists, Orthopaedic, and Trauma surgeons.
  2. 32-year-old Wade Wilson, a known case of HIV, came to the hospital with pedal edema for 2weeks. He says he was being treated for HIV when he recently started noticing foamy urine. After a renal biopsy, he was diagnosed with Nephrotic syndrome secondary to HIV. Heis now on medical treatment for the same.
  3. 57-year-old Bruce Banner came to the hospital with complaints of blurred vision for 2 months. He has been on oral hypoglycemic drugs for 10 years now as he despises insulin injections. He was diagnosed with diabetic retinopathy and is being managed by an Ophthalmologist and an Endocrinologist.
  4. 45-year-old Steve Rogers came to the hospital with generalized pain all over the body, loss of weight for 3 months, and loss of appetite. He underwent a PET CT scan and was diagnosed with a Parathyroid tumor. His blood tests showed increased levels of parathyroid hormone and Calcium. The Orthopedic surgeon has requested him to get a Bone Densitometry test done as he suspects osteoporosis secondary to hyperparathyroidism is a reason for the generalized pain. He is currently posted for surgical excision of the tumor.

  1. Logan, a 7-year-old boy who was diagnosed with cystic fibrosis at birth was accompanied to the hospital by his stepmother with complaints of high fever with chills/rigors, cough, and weakness. The stepmother claims that Logan looks malnourished because she forgets packing him breakfast. Investigations confirmed Lower respiratory tract infection and malnutrition. He is now being managed by the pediatrics department, pulmonologists, and gastroenterologists in the respiratory ICU.
  2. Sheila, a 35-year-old female who was on and off chemotherapy for 2 years for Carcinoma Cervix, came to the hospital with paralysis of the left side of the body. An MRI scan of the brain showed metastasis of the tumor to the brain and spinal cord. She is now being managed by surgical oncologists.

Description of the life stage

Describe the following information for Late Infancy 6-12 months:

1. Description of the life stage: age range, what’s happening to the body in that age range, physical milestones that occur during that age range, etc.

2. Overview of EER and macronutrient ranges

3. One specific macronutrient that is of particular interest to individuals in this stage. Provide the requirements (things like RDA, AMDR) for this macronutrient and explain why it is of particular interest in this stage.

4. Two specific micronutrients that are of particular interest to individuals in this stage. Provide the requirements (DRIs like RDA or AI) for these micronutrients and explain why they are of particular interest in this stage. The DRIs for these micronutrients must be clearly specified.

5. Ideas for at least one food source for each nutrient to help meet these needs (or explain that the nutrient need must be met with supplements if that is the case)

This means at least three foods: one source for your macronutrient, one for each micronutrient

Include the food’s typical serving size and the amount of the nutrient in that serving,

If people in your assigned age range generally require a supplement for a nutrient, explain why and describe any risks or special concerns associated with supplementation.

The warning signs of abusive and physically threatening behaviors

The case study and answer the following questions:

After caring for a client who has been in her care for the better part of two years, a community care worker/health care worker has formed an emotional bond with her. When the client passes away unexpectedly, the care worker started experiencing grief reactions, depression and mood swings, and anxiety attacks.

1. When working with clients for an extended period of time, it is normal to experience feelings of grief and loss.

2. “Grief work” is a term used to describe the process of dealing with grief.

3. What three (3) strategies may the caregiver use to help her in coping with her grief and loss?

the case study and answer the following questions:

As a care worker, you’ve been assigned a client that is extremely abusive and physically threatening to you and the rest of the staff. You’ve had previous interactions with this client, and it wasn’t a pleasant experience for anyone.

1. What are the warning signs of abusive and physically threatening behaviors?

2. In order to protect yourself and others from abusive and physically dangerous behaviors, what are the three (3) best measures to employ?

the case study and answer the following questions:

During a shift in which you are working alone while other care workers are assisting other clients, you notice that one client is yelling in pain because the nurse forgot to give him pain relief medication, and another client requires your assistance with a manual handling task that requires the assistance of another person, and a third client requires your assistance to read an official notice that must be answered.

  1. If you were to organize and prioritize your time management tasks according to the scenario described in the case study, how would you carry out the activities?
  2. How should you prioritize your tasks when everything is important?
  3. In the context of community services and/or health care, identify and explain the name of the technique used for task planning and prioritization?
  4. What are the legal rights that employees have under the Fair Work Act 2009 (FW Act)?
  5. What protections does the Fair Work Act provide to employers?

Nursing care plan for the impaired physical mobility

Make a nursing care plan for the impaired physical mobility of a 20 years old woman who had just given birth to a healthy baby boy with a 3rd-degree perennial laceration and repair. She reported pain and discomfort in that impaired area. Vital signs are normal.

NCP includes nursing diagnosis, scientific basis, nursing responsibilities, and rationale for each.

Individualized Practicum Learning Objectives

Explain two-three Individualized Practicum Learning Objectives that are SMART (i.e., Specific, Measurable, Attainable, Results-focused, Time-bound) and reflective of the higher-order domains of Bloom’s taxonomy (i.e., Application level and above) for hospital readmissions of CHF patients

Caring for the geriatric population

Conner Adams is a new graduate of a nursing program. He practiced in many different clinical sites, during his schooling. Long-term care settings were a part of his clinical rotation; he found a love for caring for the geriatric population. His mother and father have always told him that he should take the first year after schooling and start working in a hospital. They did not think that the nursing home setting was a good place to start. (Learning Objective 4.)

1.   What if…You are a good friend of Conner? What things can you explain to Conner about the role of the LPN?

2.   What if…Conner is interested in what actual nursing care responsibilities he would have. What are the major responsibilities of gerontological nurses in long-term care facilities?

3.   What if…Conner’s parents state that nursing homes are poorly run and the residents are not being taking care of. What can you tell Conner about the conditions in nursing homes today?

4.   What if…Conner’s parents tell him that older people are being placed in these nursing homes when no one wants to care for them. Explain why patients are placed in nursing homes to Conner.
