Stage 3 ulcer

Mrs Jillian is a 95 year old female admitted to long term care from Hospital after a fall at home. She was living alone. Patient appears thin and frail. She has good appetite but poor dentition – several missing teeth. She is currently on a regular diet. She also has a stage 3 ulcer on buttocks Focus, what would you do? Suggest possible changes/what to consider to help improve nutritional status.
