Utilization of limited resources to care for patients

As a Nurse what are your thoughts on utilization of limited resources to care for patients with a low probability of recovery or return to a “meaningful” life?  Who determines what “meaningful life” is for a patient?  How do Nurse manage their emotions when , after days of caring for a patient, the family chooses and the MD orders  extubation of a patient to allow them to die?
